DESCRIPTION:Listen\n        \n        \n\nThe purpose of this workshop is to educate families who have a loved one with a significant disability about the importance of planning for the individual’s future support.   Planned Lifetime Assistance Network of NJ (PLAN/NJ) will guide families in this legal and estate planning and life-long support process to answer the question\, “Who will care for my loved one when I am gone?”\n\nDescription: This session will address future planning issues such as independent living supports; estate planning\, legal and financial protections. Topics include how to implement a future life plan for people with disabilities\, Guardianship and Supported Decision Making\, Special Needs Trusts\, and ABLE accounts. She later moved on to Cranford Public Schools where she served as a Principal for 8 years before joining First Children in 2010. Kathleen is a certified school superintendent\, principal\, supervisor\, teacher of the handicapped and elementary school teacher. She holds a Master’s degree in Teaching from Marygrove College and a Master’s in Educational Administration from Kean University. Some of her accomplishments/awards are state model of the Olweus Bully Prevention program\, assistant director of the Association of Supervision and Curriculum Development-northeast region\, Principal of the Year-Cranford Public Schools 2003/2004 and NCASES board member and commissioner. As a certified mediator Kathleen has been appointed by the New Jersey Supreme Court to be a fee arbitrator for attorney conflicts. Kathleen is a passionate educator and visionary of academic excellence for all children.\n\nTara Szymanek is a trainer and Family Resource Specialist at SPAN Parent Advocacy Network. She is a military spouse who provides her support for military families with dependents with disabilities. She enjoys personal growth and development\, developing\, providing training\, public speaking\, advocating in person and in writing. DESCRIPTION:Listen\n        \n        Starting at age 14\, a student’s IEP must include a Transition Plan. According to the New Jersey Department of Education the purpose of this plan is to “prepare students for a successful entry into adult life after high school”. The Transition Plan is the roadmap to adulthood for students with IEPs. Families need to develop a clear vision of the destination so they can determine the best route to their loved one’s adult life destination. Most importantly families need to feel empowered to articulate that vision to the school team.\n\nParticipants in this forum will gain an understanding of:\n\n 	The components of a good Transition Plan\n 	Graduation date determinants\n 	How to create a vision for a successful adult life\n 	How to create a person-centered plan for your loved one\n 	The components of an assessment for transition\n 	Developing IEP goals and objectives based on your vision and assessment data\n\nFamilies and students are equal members of the Child Study Team. AM session (10AM to 12PM)\nNJ FamilyCare/Medicaid and Individuals with IDD: Preparing for the Start of Medicaid Redeterminations\nWhen the COVID pandemic began in March\, 2020\, the federal government declared a Public Health Emergency (PHE)\, which included “continuous Medicaid enrollment”\, i.e.\, everyone who already had Medicaid was required to continue receiving Medicaid — even if they were no longer eligible. The “continuous Medicaid enrollment” requirement will be ending in April\, 2023\, at which time NJ Medicaid will start to send Medicaid redetermination applications to everyone who has NJ FamilyCare/Medicaid. Those applications will be mailed out over a period of 12 months. Whenever a caregiver of an individual with an intellectual or developmental disability (IDD) receives a letter from NJ Medicaid\, it is very important to respond promptly. Please note: The information on Medicaid redeterminations pertains to people who have NJ FamilyCare/Medicaid. This information is not applicable to people who have SSI and Medicaid. \nJoin us on March 25 from 10AM to 12PM for information on Medicaid and different Medicaid categories in New Jersey. You will learn what you need to do now so that the Medicaid redetermination process goes smoothly. We will discuss potential areas of confusion and what to do if you encounter a problem with your loved one’s Medicaid redetermination. Beverly Roberts\, Director\, Mainstreaming Medical Care at The Arc of New Jersey will be our guest presenter and will answer your questions. We will also be joined by Lee Ginsberg from Community Health Law Project and Michael Brower from Disability Rights New Jersey to discuss the appeal process.
