According to the New Jersey Department of Education the purpose of this plan is to “prepare students for a successful entry into adult life after high school”. The Transition Plan is the roadmap to adulthood for students with IEPs. Families need to develop a clear vision of the destination so they can determine the best route to their loved one’s adult life destination. The “continuous Medicaid enrollment” requirement will be ending in April\, 2023\, at which time NJ Medicaid will start to send Medicaid redetermination applications to everyone who has NJ FamilyCare/Medicaid. Those applications will be mailed out over a period of 12 months. Whenever a caregiver of an individual with an intellectual or developmental disability (IDD) receives a letter from NJ Medicaid\, it is very important to respond promptly. Please note: The information on Medicaid redeterminations pertains to people who have NJ FamilyCare/Medicaid.
