SUMMARY:Person-Centered Life Planning | Planificación de la vida centrada en la persona
DESCRIPTION:Listen\n        \n        \n\nThe purpose of this workshop is to educate families who have a loved one with a significant disability about the importance of planning for the individual’s future support.   Planned Lifetime Assistance Network of NJ (PLAN/NJ) will guide families in this legal and estate planning and life-long support process to answer the question\, “Who will care for my loved one when I am gone?”\n\nDescription: This session will address future planning issues such as independent living supports; estate planning\, legal and financial protections. Topics include how to implement a future life plan for people with disabilities\, Guardianship and Supported Decision Making\, Special Needs Trusts\, and ABLE accounts. SUMMARY:Person-Centered Planning Meeting with Michael Smull
DESCRIPTION:Listen\n        \n        \n\nMichael Small is the co-developer of person centered thinking training and of essential lifestyle planning. Mr. Smull has provided training and consultation in 49 states\, (including New Jersey) and 8 countries. He has assisted agencies\, states\, regions\, and counties to make the changes needed to support person centered practices. Mr. Smull has written extensively on person centered thinking and planning\, the challenge of changing to a person centered system\, and issues relating to supporting people with challenging behaviors. The person centered thinking framework that he and his colleagues developed is reflected in the Home Community Based Service (HCBS) final rule.
