DESCRIPTION:Listen\n        \n        Starting at age 14\, a student’s IEP must include a Transition Plan. According to the New Jersey Department of Education the purpose of this plan is to “prepare students for a successful entry into adult life after high school”. The Transition Plan is the roadmap to adulthood for students with IEPs. Families need to develop a clear vision of the destination so they can determine the best route to their loved one’s adult life destination. Most importantly families need to feel empowered to articulate that vision to the school team.\n\nParticipants in this forum will gain an understanding of:\n\n 	The components of a good Transition Plan\n 	Graduation date determinants\n 	How to create a vision for a successful adult life\n 	How to create a person-centered plan for your loved one\n 	The components of an assessment for transition\n 	Developing IEP goals and objectives based on your vision and assessment data\n\nFamilies and students are equal members of the Child Study Team.
