Hospital Visitation Policy
Hospitals are required to allow a designated support person to be with’ the disabled patient, in both the emergency room and during hospitalization, regardless of the reason for the hospitalization, for patients for whom a support person has been determined to be medically necessary.
Nominations can be submitted electronically here or by completed the nomination form. Access the Nomination Form Here. You can request assistance in completing the nomination form by emailing NJCDD@NJCDD.org. The Application Deadline Extended to July 8, 2022
